About Us



Our Location

Our purpose and mission is to improve our patients' quality of life. We will guide and educate them through all areas necessary to achieve a balanced life that is satisfying to them as individuals and members of society.

As part of our goals, our practice offers a comprehensive approach for the treatment of chronic pain and palliative care. It will be guided by the principles of honesty, integrity, and ethical services to our patients. Our ultimate purpose is the total well-being of body, mind, and spirit.

Those who visit Dr. Euribe will likely become familiar with his seven-year-old yellow Labrador, Max. Dr. Euribe takes Max to work every day.

Appointments

We are a referral based clinic, which means you need a referral from your physician in order to see us. We do not accept self referrals or walk-ins. We will schedule appointments from Monday through Friday, 8:00 am to 4:00 pm. Please make sure to provide us updated telephone information in case we need to contact you to move your appointment for any reason. If you are unable to keep your appointment, please try to give us 24 hours notice. Should you request to reschedule, we will offer you the next available opening.


Medical Records

Your records are confidential information. If you wish to request copies of your records, please allow 72 hours before the copies will be ready for pick up. If you will be having someone else pick up the records for you, please send us a written request authorizing us to release your records to that person. (They will be required to show us identification) Please keep our office informed of any changes in your name, address, telephone number, and insurance information.


Prescription Refills

If you call the office requesting a prescription refill, please be aware that all telephone messages are reviewed by the providers at the end of the day once they are done with clinic. We will call you back at that time to give you instructions on your refill or we will call the pharmacy as needed. If you call after hours or on weekends you will be directed to our answering service. After hours, doctors will be paged for emergency situations only. It is our policy not to refill prescriptions on weekends or nights. If your emergency is such, you will need to go to the nearest emergency room.

Please remember to include your name, telephone number, pharmacy name and telephone number, drug name, strength and dosage when requesting your refill.


Insurances

We participate with most insurance plans, including Medicare, BCBS, Freedom Health, Health Choice, United Health, and PUP. If you have a co-pay or will be responsible for a percentage of your bill, please be aware that payment is expected the day of your visit.

We do not accept Letters of Protection.

If you have a secondary insurance we will file it as a courtesy, but will balance bill if the insurance does not pay for any reason.
