yourself with our guidelines. Prior to your appointment at our office,
you will need to print out and complete the new patient questionnaire
packet which can be found on the New Patients page.
It is also very important that you bring with you a list of medications you are currently taking, reports of the most recent X-rays, MRIs, or cat scans, and notes from prior consultations you may have had with other physicians concerning the problem you are seeing us for.
Please be aware that your first visit to our clinic is only an evaluation and you will not be getting an injection on this day.
The providers at Central Florida Pain Management believe in comprehensive pain management which may include any of the following:
Physical therapy
Interventional pain management
Pain medication management
Random drug testing
Mental health evaluation by a psychologist
To avoid any charges for missed appointments, kindly gives us 24-hour notice of cancellation.
